Birth certificates available in Page today and Oct. 7

PAGE, Ariz. - The Coconino County Health Department (CCHD) will provide birth certificate services in Page today and again on Wednesday, Oct. 7. The service will be offered from 10 a.m. -2 p.m. at the CCHD Northern Region office located at 467 Vista Ave.

The CCHD can only provide birth certificates for births that occurred in Arizona; however, Vital Records Office contact information for other states will be provided. The cost for a birth certificate is $15 that can be paid by cash, money orders, cashier's check, and personal checks.

Requirements:

• Requests for birth certificates must be made in person.

• Applicants must be 18 years of age or older, provide unexpired Government-issued identification, and in certain cases, prove relationship to the registrant if requesting for a birth certificate for someone other than themselves.

• Births that occurred from 1950-1989 will be issued 'as is' and will not be adequate for use to obtain a passport or travel outside of the country. These applicants may request a complete birth certificate from the State Office of Vital Records.

• For births that occurred prior to 1950, applicants can only receive their birth certificate through the State Office of Vital Records, not through the Coconino County Health Department.
